Serhiy Ihorovych Kozachuk was born on November 27, 1971, in Rivne. He studied at Rivne General Education School No. 16 and obtained the profession of electrical device settings technician at Rivne Vocational Technical School No. 5.

At the start of the war, Serhiy did not wait for a summons and on the second day went to the recruitment center independently. On March 6, 2022, he joined the Armed Forces of Ukraine. Unfortunately, Sergeant Serhiy Ihorovych died in the Pokrovsk direction on May 8, 2024. Only now, after the DNA examination, was his body returned for a dignified farewell.

The farewell to the defender took place on June 4 at Independence Square in Rivne. The sergeant was buried at the 'New Yard' cemetery.
