Titan Tragedy: Investigation Reveals Preventable Causes of the Disaster.
The U.S. Coast Guard investigative panel concluded that the 2023 tourist submarine Titan disaster could have been prevented. The report highlights serious safety and operational issues with OceanGate, the company that built the submarine, and calls for increased oversight of new vessels.

Investigation Findings
According to the document, which spans approximately 300 pages, 'poor design, certification, maintenance, and inspection of the Titan' were key factors in the tragedy. It also emphasizes 'toxic corporate culture at OceanGate,' inadequate regulation of submarines and other new vessels, as well as ineffective mechanisms for controlling violations.
Board chairman Captain Jason Neubauer stated that the crash could have been avoided had there been proper oversight and clear rules for operators testing new concepts outside the conventional frameworks.
During the tourist expedition, the Titan vanished into the ocean abyss while descending to the wreckage of the Titanic, losing contact with other vessels. After extensive searches, the wreckage was discovered on the seabed near the liner that sank in 1912.
Following the incident, OceanGate suspended all operations and pledged full cooperation with the investigation. It was noted that prior to the tragedy, the company had been avoiding regulatory scrutiny, citing scientific missions and a positive reputation.
The daughter of one of the deceased in the vessel expressed satisfaction with the investigation's results.
'The head of OceanGate did not act appropriately, and this had its consequences. This tragedy was foreseeable, not a result of chance. It could have been avoided.' — she stated.
The report also noted issues with oversight and deficiencies in OceanGate's operations prior to the incident. It was determined that oversight and cooperation among various agencies were inadequate, leading to the disaster.
